Urology

Difficult Foley

  • Urojet and 18 fr coude is go to
  • Can try downsizing for stricture
  • If getting blood back from difficult placement, probably time to stop and talk to urology

Prostatitis/Complicated UTI

  • Always send culture
  • 2 weeks of antibiotics probably reasonable starting point with urology follow up for further evaluation and consideration of the full 4-6 wk course

Priapism

  • Blood gas from cavernosal blood is helpful for urology evaluation

Renal Colic

  • For most 2-3 week follow up is probably appropriate b/c most people will get 4-5 week trial for passage
  • For stones ≥ 8 mm probably talk to urology b/c unlikely to pass spontaneously (~ 20% chance)
  • 5 mm stones ~50% passage, 6 mm ~40%, 7 mm ~30%
  • Consider KUB if sending stone home so that if stone is radioopaque it can be followed with just Xray
  • All people with stone need to be followed to passage – pain not always entirely reliable to prove passage, need to be seen in f/u even if symptoms resolve
