Persistent Fever

e.g. > 5 days

DDx

  • Kawasaki
  • Malignancy
  • Osteomyelitis
  • Bartonella
  • Bacteremia
  • UTI
  • MAS/HLH
  • Rheum

Kawasaki Disease

  • Clinical features:
    • Fever > 38 C x 5 d
    • Conjunctival injection (limbic sparing, non-exudative)
    • Rash: Polymorphous non-vesicular
    • Mucosal changes (lips and tongue)
    • Cervical LAD > 1.5 cm
    • Edema/desquamation palms/soles
  • If all 5 clinical features present can dx after 4 days of fever
  • Fever > 7 days in < 6 mo should be considered Kawasaki until proven otherwise
