SBP

• Dx > 250 PMNs
• Tx ceftriaxone 1 g or cefoxitin 2 g
• Albumin 1.5 g/kg (that’s a high dose) if:
  • Creat >1
  • BUN >30
  • Bili > 4

Variceal Bleed

• Massive transfusion
• Fibrinogen
  • Target > 150 or
  • Empiric 10 units cryo (each unit is 10-15 ml and raises fibrinogen ~7)
• Octreotide – 50. mcg bolus then drip
• Ceftriaxone 1 g
• MN Tube or Blakemore

Large Volume Paracentesis

• 6-8 g/L of fluid removed for paracentesis of ≥4-5 L
